Home Language Survey

In accordance with Performance Based Accreditation guidelines, all schools are required to administer a Home Language Survey (HLS) to identify the first (native) language(s) of all students enrolled in the school corporation. The Home Language Survey shall elicit the following information:

  •  the first (native) language of the student;

  •  the language most often spoken by the student;

  •  the language most often spoken at home.

The Home Language Survey should be given to any family that is registering for school for the first time in Indiana. Documentation of a student’s native language shall be recorded in the student’s permanent record and a copy of the HLS will be in the student's permanent file. If any language other than English is on the HLS, a copy of the HLS should be forwarded to the Office of Academic Programs so that the need for English language Proficiency testing may be determined. If we receive a transfer student, the building should secure the original home language survey from the last school. The Home Language Survey form is available on the school’s website (http://alexandriacsc.schoolinsites.com) by clicking on the link to the Office of Academic Programs.
